Jason Edward Baker, 33, of Windsor formerly of Shelbyville passed away at 11:52 p.m. Tuesday at the Sarah Bush Lincoln Health Care Center, Coles County.

Funeral services will be held at 2:00 p.m. Friday in the Lockart-Green Funeral Home, Shelbyville with Pastor Ron Brown officiating. Visitation will be from 11:00 a.m. until service time. The family requests casual attire. Burial will be in Oak Grove Cemetery near Cowden. Memorials may be made to the family.

He was born September 5, 1981 in Decatur the son of Sandi Koshinski and Jesse Baker.

He was involved with Mixed Martial Arts and was an avid Chicago Bears and St. Louis Cardinals fan.

Surviving are his parents, Jesse and Sandi Baker of Shelbyville; daughters, Amber Lynn Ford (Baker) and Leslie Dawn Ford (Baker) of Shelbyville; brother, Chad Eugene Baker and wife Michelle of Guthrie, OK; sisters, Melissa Dawn Baker of Phoenix, AZ and Jessica Carol Baker and Cohen Bray of Shelbyville; grandmother, Ruby Lorene Hicks of Lakewood; girl friend, Michelle Harris and her son Gavin Donald of Windsor, nephew Zayden Bray of Shelbyville; niece, Tiffany LaFerry of Guthrie, OK; several aunts, uncles and cousins.

He was preceded in death by his grandparents, Robert and Donna Koshinski and Cletis Hicks and baby sister Christy Koshinski.
